Part 2 of 2: Respirators- Medical Clearance

Friday covered what a respirator asks of your body. Medical clearance is how the job confirms your body can handle it, before you are fit tested and before you wear one on the job.
Who does it. A physician or other licensed health care professional (PLHCP), using the mandatory OSHA questionnaire in Appendix C or an exam that gets the same information.
What it asks. Every worker selected to wear a respirator answers the core questions: tobacco use; conditions such as seizures, diabetes, breathing allergies, claustrophobia, and trouble smelling odors; lung and heart history and symptoms; medications; and past problems with a respirator. Full-facepiece and SCBA users answer added questions on vision, hearing, and back and muscle problems.
Your answers are private. The questionnaire tells the employer not to look at or review your answers. It is offered during normal working hours, or at a time and place convenient to you, at no cost to you.
What happens next. A "yes" to any of questions 1 through 8 triggers a follow-up medical exam. The employer gets only a written recommendation: any limits on respirator use, whether follow-up is needed, and a statement that you received a copy. If a condition makes a negative-pressure respirator risky but the PLHCP clears a powered air-purifying respirator (PAPR), the employer provides a PAPR.
It is not once and done, but it is not annual either. Another evaluation is required when you report symptoms, when the PLHCP, a supervisor, or the program administrator asks for one, when fit testing or other program information points to a problem, or when working conditions change in ways that add real physiological burden, such as work effort, protective clothing, or temperature.
Watch the rulebook. OSHA has proposed dropping the medical evaluation requirement for filtering facepieces and loose-fitting PAPRs (proposed July 1, 2025). Its construction advisory committee voted 5 to 4 in April 2026 to recommend against it. As of this edition it remains a proposal, and the requirement stands.
Key Takeaways:
• Medical evaluation comes before fit testing and before use, at no cost to the worker.
• Answers go to the health care professional, not the employer.
• A "yes" on questions 1 through 8 means a follow-up exam; a PAPR is an option when the PLHCP clears it.
• Report new symptoms or changed conditions; that triggers a new evaluation.
